"From this year, application forms would be received only from those who have never performed Haj in their lifetime," said SHC chairman Syed Khaleeluddin Ahmed.
The Haj application forms for Haj 2013 will be issued at the AP State Haj Committee Office, and at the District Haj Societies free of cost from February 6 to March 20.
The forms can also be downloaded from the website of the Haj Committee of India www.hajcommittee.com and submitted the same way along with necessary documents on or before March 20 at the office of State Haj Committee at Haj House in Nampally between 10:00 AM to 3:00 PM on all working days.
SHC chairman Syed Khaleeluddin Ahmed and Executive Officer Abdul Hameed in a statement said that from this year, application forms would be received only from those who have never performed Haj in their lifetime through the Haj committee for which all the applicants will have to submit a notarised affidavit to this effect.
They said that all the applicants are required to submit compulsorily the required documents along with filled in application forms, i. e., a self-attested copy of the passport issued on or before March 20, 2013, and valid up to March 31, 2014, a copy of ID and address proof; a canceled blank bank cheque with IFS Code, a pay-in-slip of SBI for Rs. 300 per applicant, and a medical screening and fitness certificate for each applicant.
They further said that the applicants under Reserved Categories should submit their original passports along with a notarised affidavit in the format given in the Haj Application Forms on a stamp paper of Rs 10. (INN)
